Yes, believe it or not, it's happened before. The wrong girl has been mistakenly crowned as the winner of the pageant. Here are three times we can recall:
1. Miss Universe Canada
In 2013, Denise Garrido was wrongly named winner of Miss Universe Canada. Unlike Steve Harvey, who immediately corrected his mistake at Miss Universe 2015, Garrido wasn't told until an entire day later.
After releasing a statement, Garrido gave up the crown and sash to its rightful owner Riza Santos. The error was made in the score card tabulation and recounted, finding that Garrido had actually come in 4th place out of 57 contestants.

2. Miss Nevada's Outstanding Teen
Amy Smith was crowned Miss Nevada's Oustanding Teen in 2014. The correct winner was Haleyann Hart. After a legal battle, it was concluded that two Miss Nevada Teens would be sent to the national competition in Oranlado that June.
That night was already interesting enough, after Emilee Smith, Amy's sister, was crowned Miss Nevada. That was the first time two sisters had held twin titles at the same time.

3. Miss World Puerto Rico
In 2000, the Miss World Puerto Rico competition took an interesting turn when the first runner up was wrongly crowned as the winner.
The host had actually called out the right name, but the ushers crowned the wrong girl. The host stood there trying to correct the mistake for a while, until finally the first runner up took the crown off of her own head and gave it to Toa Alta, the rightful winner.
